Wound Infection
Short answer
Pain/grunting and blur after a wound was allowed to close or treated with unsafe material strongly points toward wound infection.

What to do
Stop any remaining bleeding. If very early, use the current verified wound-disinfection route. If the infection has progressed, use the current verified antibiotic route. Keep health, water and energy stable while treating.
How it works
Wound infection is caused by poor wound hygiene rather than by the original source of the cut. Early and later stages have different treatment logic, so the site must not collapse them into one instruction.
Don't
Letting a bleed close on its own and assuming the problem is finished. Using random pills before checking whether the infection is still in its early wound-cleaning stage.
