Short answer
Health is direct survivability. Serious damage, disease and environmental hazards can reduce it; zero health means death.

What to do
Stop the cause of health loss. Treat active bleeding and immediate injuries. Stabilize temperature, food and hydration. Diagnose disease if health continues dropping.
How it works
Health is only one of several player-state systems. A character can have non-zero health but still collapse because of shock or severe blood loss.
